Multiple Bier blocks with labetalol for complex regional pain syndrome refractory to other treatments.

چکیده
To the Editor: Although the Bier block has been used as a treatment for sympathetically-maintained pain (SMP) for many years, the selection of agents remains controversial. Different drug classes have been used, including alpha-adrenergic blockers (guanethidine, reserpine, bretylium), alpha-2 agonists (clonidine), nonsteroidal antiinflammatory agents (ketorolac, tenoxicam), local anesthetics (lidocaine, prilocaine), and NMDA antagonists (ketamine). To our knowledge the use of labetalol for Bier block has not been reported. We here report a case of complex regional pain syndrome (CRPS) that did not respond to a standard Bier block with bretylium and lidocaine, but responded for 7 years to multiple Bier blocks with labetalol and lidocaine.

Continuous peripheral nerve blocks at home for treatment of recurrent complex regional pain syndrome I in children.
BACKGROUND Recurrent complex regional pain syndrome I is not rare in the pediatric population. The authors conducted this study to evaluate the efficacy of continuous peripheral nerve blocks with elastomeric disposable pumps associated with initial Bier blocks for the treatment of recurrent complex regional pain syndrome I in children. METHODS After parental informed consent, 13 children who ...

متن کاملComplex Regional Pain Syndrome Revived by Epileptic Seizure Then Disappeared Soon during Treatment with Regional Intravenous Nerve Blockade: A Case Report
We present a case of complex regional pain syndrome (CRPS), in which symptoms, including burning pain and severe allodynia, were alleviated by using a regional intravenous nerve blockade (Bier block) combined with physiotherapy, but reappeared following an epileptic seizure. Symptoms disappeared again following control of epileptic discharges, as revealed by single-photon emission computed tomo...

Complex regional pain syndrome (CRPS) type I is a painful and disabling syndrome that is accompanied by physical changes in the affected extremity. It generally occurs after trauma, manifesting as pain that is out of proportion to the inciting event. Treatment of the disorder is difficult, with many patients being refractory to multiple pharmacologic regimens. Regional anesthetic techniques, in...
متن کاملComplex regional pain syndrome: observations on diagnosis, treatment and definition of a new subgroup.
Several definitions and sets of diagnostic criteria of complex regional pain syndrome have been proposed, but to date none has been accepted completely. This article presents a specific subtype of the disease, called 'chronic, refractory complex regional pain syndrome' which is extremely severe, disabling and resistant to treatment. It also emphasizes difficulties with diagnosing complex region...
